The purpose of this form is prevent leaders in Fusion Student Ministry from posting pictures of your student(s) on social media. You have our commitment that your student(s) will NOT be excluded from any activities at Fusion Student Ministry and will NOT be treated any differently if we are to receive this form. We respect your authority as their parent or guardian and your choice to protect your student's privacy!
